An intramedullary implant has opposite ends inserted into bores in abutting phalanges for correcting hammer toe and for similar arthrodesis procedures. A rear end of the implant may be threaded and is received in a bore in the proximal phalanx. A front end has an asymmetric pointed shape with longitudinally and/or angularly spaced rear-facing gripping flukes. The pointed front f4 end is received in the middle phalanx. The asymmetric pointed shape is self-aligning, enabling entry to begin without first longitud inally aligning the bones. The rear end when screwed into place can be rotated to select the orientation of the asymmetric pointed shape. The pointed front end is inserted and pushed to bring the phalanges into abutment where the implant holds them against re traction or rotational displacement.

The asymmetric pointed shape is self-aligning, enabling entry to begin without first longitudinally aligning the bones. In one embodiment, the invention includes a method that uses images such as radiographs preoperatively to determine the length and width of the disclosed implant or any other implant based on each patient's unique anatomy, that will properly allow coaptation of the ends of the prepared tibia and talus at the ankle joint or the prepared talus and the calcaneus at the subtalar joint. The implant is inserted from the bottom of the foot through a predetermined hole in the calcaneus extending through the talus into the diaphysis of the tibia. When properly seated in the bones, the implant is locked to the bones by screws.

The invention comprises a method for fixating bones in the foot by aligning the bones in their desired position, inserting a screw in the aligned bones, inserting at least one transverse element near the head or tip of the screw, and tightening the screw to compress the bones. The screw comprises a shaft having first and second ends with spirally wound screw threads beginning near the first end and extending along the shaft. Advantageously, the screw is cannulated and screw threads are formed on an interior surface of the cannulation. Illustratively, the transverse elements may be staples, open-ended washers, or open-ended nuts. An anchorage in tissue is produced by holding a vibrating element and a counter element against each other such that their contact faces are in contact with each other, wherein at least one of the contact faces includes a thermoplastic material which is liquefiable by mechanical vibration. While holding and then moving the two elements against each other, the vibrating element is vibrated and due to the vibration the thermoplastic material is liquefied between the contact faces, and due to the relative movement is made to flow from between the contact faces and to penetrate tissue located adjacent to outer edges of the contact faces. For liquefaction of the thermoplastic material and for displacing it from between the contact faces, no force needs to act on the tissue surface which is to be penetrated by the liquefied material.

A method for compressing at least a first and second bones comprising:aligning the first and second bones in a desired alignment;inserting a screw member into a borehole extending through the aligned first and second bones, the screw member comprising an elongated body extending from a first end to a second end, wherein the first elongated body includes threads near the first end and a head at the second end;inserting a first transverse element into the first bone or the second bone transversely to a length of the screw member at a first position thereby engaging the elongated body of the screw member such that the screw member can be rotated with respect to the first transverse element; androtating the screw member to compress the first and second bones.2. The method of claim 1 , wherein the first and second bones that are compressed are in a human foot.3. The method of claim 1 , wherein step of aligning the first and second bones comprises the step of: inserting a guide wire through the first and second bones.4. A prosthesis suited for orthopedic implantation comprising a modular intramedullary rod that provides intramedullary guidance and that supports an artificial joint surface that can articulate with another artificial joint surface in various ways. Intramedullary guidance systems and methods introduce some and/or all surgical tools and ankle prostheses components through the tibia, using minimal invasive exposure. The systems and methods align the talus and tibia for the installation of one or more ankle prostheses components, and also maintain that alignment during the installation using intramedullary guidance. 1. An intramedullary rod , comprising:a plurality of modular components sized and configured to be disposed in an intramedullary canal;wherein said modular components are configured to be connected to each other in situ; andwherein the distal-most modular component is a base component configured to engage an alignment guide.2.
