A surgical filament snare assembly including an anchor capable of being fixated in bone and having a filament engagement feature. A first filament has a noose with first and second noose limbs connected, preferably slidably, to the filament engagement feature. The first and second noose limbs emerge from the anchor as first and second free filament limbs that are capable of being passed through tissue to be repaired and then passable through the noose. The noose is capable of receiving the free filament limbs and strangulating them when tension is applied to at least one of the free filament limbs and the noose to enable incremental tensioning of the tissue after anchor fixation. Preferably, the snare assembly further includes a flexible sleeve joining at least some portion of the first and second free filament limbs to facilitate passing of the free filament limbs as a single unit.

An external guide catheter can be inserted through a vein of a patient into the right ventricle via the right atrium. A surgical construct for tissue repairs , comprising:a tensionable construct comprising a flexible strand with a plurality of limbs, and a shuttling device attached to the flexible strand,wherein the shuttling device allows the plurality of limbs of the flexible strand to pass through the flexible strand and form a locking splice and multiple adjustable closed loops.2. The surgical construct of claim 1 , wherein the tensionable construct consists of the flexible strand with the plurality of limbs and the shuttling device attached to the flexible strand.3. A method for securing suture to bone comprises drilling a hole in a desired portion of bone at a desired procedural site, passing a strand of suture through a portion of soft tissue to be approximated to the portion of bone, and extending the free suture ends proximally from the soft tissue. The suture is loaded into an anchor implant. Using an inserter, on a distal end of which is attached the anchor implant, the anchor implant is manipulated into the bone hole. The suture is then tensioned to a desired level by pulling on the free suture ends, after the anchor implant is positioned in the bone hole. The free suture ends are wrapped about a suture cleat on an inserter handle once the desired tension level is achieved. The present invention provides a surgical system for securing a suture against relative movement with respect to a body tissue. The surgical system includes a fastener and medical device for positioning and securing a fastener onto a suture. The fastener includes inner and outer members that can slide together and rotate. The suture extends through the inner and outer members so that rotation of the inner member causes it to wrap the suture around it. As the suture is wound around the inner member, the fastener is drawn toward a position against the body tissue. Further rotation of the inner member allows tension to be applied to the suture. There is provided a suture anchor for soft tissue repair. The anchor is substantially elongate so as to be adapted for inserting into a blind hole or socket created in bone in use. The anchor defines externally and outwardly operative engaging means adapted for engaging the inner surface of the blind hole or socket for securing the anchor within the blind hole or socket in use. The anchor defines a distally located suture engagement adapted for engaging a suture in use, wherein, in use: the anchor is adapted for insertion into the blind hole or socket, such that: the engaging means is adapted for securing the anchor within the blind hole or socket; the engaging means is adapted for frictionally engaging the suture between the outer surface of the engaging means and the inner surface of the blind hole or socket; and the suture engagement is adapted for engaging the suture so as to substantially prevent the inadvertent disengagement of the suture from the blind hole or socket caused by the sliding of the suture between the outer surface of the anchor and the inner surface of the socket. 1. Devices and methods used in termination of a tissue tightening procedure are described. Termination includes the cinching of a tether to tighten the tissue, locking the tether to maintain tension, and cutting excess tether. In procedures involving anchors secured to the tissue, the tether is coupled to the anchors and the tissue is tightened via tension applied to the anchors by cinching the tether. In general, the devices and methods can be used in minimally invasive surgical procedures, and can be applied through small incisions or intravascularly. A method for tightening tissue by fixedly coupling a first anchor to a tether and slidably coupling a second anchor to the tether, securing both anchors to the tissue, applying tension to the tether intravascularly, fixedly coupling the tether to the second anchor, and cutting the tether is described. The tissue to be tightened can comprise heart tissue, in particular heart valve annulus tissue. Various devices and methods for locking the tether in place and cutting excess tether are described.
