The electrode structure carries a sensor at a known location on the electrode structure to monitor an operating condition. A device and method for treating female urinary incontinence and also for treating fecal incontinence are presented. A fractional RF grid array is used to heat target tissue to a temperature of about 40-45° C. for a specified period of time, thus stimulating fibroblasts to produce type 1 collagen preferably without producing significant amounts of type 3 collagen. The device includes a temperature sensor and a control circuit to maintain the desired temperature of the target tissue and avoid overheating. Target tissue includes tissue in and around the urethra, and in and around the anus including the perineum. The wand to which the RF grid array is mounted has visible gradations on it so that the user can treat one area at a time, then reliably move the grid array to the next adjacent area even though the grid array is hidden from view inside a portion of the body. The controller can determine when the treatment is completed based on time, temperature, tissue impedance or any combination thereof.

The invention provides improved devices, methods, and systems for shrinking of collagenated tissues, particularly for treating urinary incontinence in a noninvasive manner by directing energy to a patient's own support tissues. This energy heats fascia and other collagenated support tissues, causing them to contract. The energy can be applied intermittently, often between a pair of large plate electrodes having cooled flat electrode surfaces, the electrodes optionally being supported by a clamp structure. Such cooled plate electrodes are capable of directing electrical energy through an intermediate tissue and into fascia while the cooled electrode surface prevents injury to the intermediate tissue, particularly where the electrode surfaces are cooled before, during, and after an intermittent heating cycle. Ideally, the plate electrode comprises an electrode array including discrete electrode surface segments so that the current flux can be varied to selectively target the fascia. Alternatively, chilled 'liquid electrodes' may direct current through a selected portion of the bladder while also cooling the bladder wall, an insulating gas can prevent heating of an alternative bladder portion and the adjacent tissues, and/or ultrasound transducers direct energy through an intermediate tissue and into fascia with little or no injury to the intermediate tissue.

By selectively contracting the support tissues, the bladder neck, sphincter, and other components of the urinary tract responsible for the control of urinary flow can be reconfigured or supported in a manner which reduces urinary leakage.

An ablation apparatus has an expandable member that is inserted into an organ of a body and ablates all or a selected portion of the inner layer of the organ. Electrolytic solution fills the expandable member, and the expandable member includes a plurality of apertures from which electrolytic solution flows from the expandable member. First and second fluid conduits, which can be first and second conforming members, are in a surrounding relationship to the expandable member. The second conforming member, including a conductive surface, is made of a material that provides substantial conformity between the conductive surface and a shape of the inner layer of the organ. A plurality of electrodes is positioned between the two conforming members. The expandable member serves as an insulator to RF energy. Each electrode includes an insulator formed on a surface of the electrode positioned adjacent to the second conforming member. The combination of sandwiching the electrodes between the two conforming members, and the use of two insulators, one on the electrode and the other on the expandable member, provides selectable ablation of the inner layer of the organ. A feedback device is included and is responsive to a detected characteristic of the inner layer. The feedback device provides a controlled delivery of RF energy to the electrodes.

A second, longitudinally movable wire passes through the lumen and is connected at the proximal end to a finger-operated slide mechanism while its distal end passes through a small aperture formed in the tube just proximal of the flexible conductive segment. That wire is then anchored distally of the flexible conductive segment so that when a tensioning force is applied to the wire, the distal end portion of the instrument will bow, with the exposed wire comprising the active electrode and the flexible conductive segment functioning as the return electrode. A two-piece handle assembly permits the sphinctertome to be treated as a disposable, single-use surgical instrument.

The energy delivery devices have relatively flexible portions near the distal ends such that when extended through the diverting region of the guide channels they are deflected radially in a direction to intersect the circumference of the biological lumen. An adaptor is coupled with the energy delivery device to supply energy for transmission to the tissue at the distal end of the device. Thus, the energy delivery device may be positioned in a withdrawn position in the catheter so that the distal end of the energy delivery device lies within the guide channel, and in an extended position so that the distal end of the energy delivery device is diverted by the diverting region of the guide channel and extends into the tissue to be treated.
