The collar piece is coupled to the shaft piece by a joint head in a ball-jointed manner. The articulation surfaces between the joint cavity in the shaft piece and the articulation head on the collar piece are embodied such that on pressing the articulation head to the base of the joint cavity, edges or projections on the one articulation surface digs into the other articulation surface lying on a virtual spherical surface. It is thus possible to achieve a ball-joint like articulation, whereby the spherical surfaces may have a relatively large production tolerance without the above affecting the clamping connection between the articulation surfaces.

An expandable intervertebral cage apparatus includes a pair of cage components, each generally in the shape of a half cylinder. Each of the cage components provides a corresponding, abutting conically shaped recess that cooperates with a conical end portion of an expansion screw. One of the cage components carries a fitting with an internally threaded bore that receives external threads of the expansion screw. As the expansion screw advances into the cage, the conically shaped end portion of the expansion screw engages the conically shaped recesses on the cage components to expand the two cage components apart until they are in contact with the vertebral plates of adjacent vertebrae. The cage components have passageways for packing with bone-inducing-growth material allowing the cage to become part of a spinal fusion. A surgical method for installing the expandable intervertebral cage is also disclosed.

Functional two-part and three-part inventive embodiments are provided, and both prostheses are similar in that only dorsoventral movement deflections are possible due to the latero-laterally oriented cylindrical convexities and corresponding concavities. In another embodiment, the cylindrical articulation surfaces are transversally curved toward the ventral region whereby enabling a minimal rotation of the sliding partners relative to one another. The intervertebral disc endoprostheses are also suited for being implanted laterally, in particular, during revision operations.

Methods and instrumentation for implantation of an intervertebral disc prosthesis are provided. A guide tool with sagittal and coronal reference features guides placement of guide pins relative to a sagittal plane and a coronal plane of the vertebrae. A retainer lockable to the guide pins has a ratcheting mechanism providing distraction or compression. A rasp, planer or other tools may be aligned to the retainer and inserted into the intervertebral space to prepare the vertebral endplates. An interoperatively adjustable trial is alignable to the retainer and measures an angularity of the intervertebral space. A prosthesis with keying features may be mounted to a prosthesis inserter with complementary features, held by pivotable arms and compressed between prongs. The inserter may be aligned to the retainer and advanced to implant the prosthesis in the intervertebral space. A prosthesis remover may grip one prosthetic end plate and extract the entire prosthesis.
