Methods and devices are disclosed for providing therapeutic hypothermia using high flow irrigation of the aerodigestive tract with cooled liquids. The disclosed method provides much more rapid therapeutic cooling to lower temperatures than could previously be achieved, thereby improving clinical outcomes. Novel cooling liquids and cooling devices are disclosed for carrying out the method. An external turbulent flow of cooling liquid may also be applied to the exterior of the head to further promote heat exchange. Multiple embodiments of devices are disclosed for performing rapid induction and maintenance of therapeutic hypothermia either in a hospital setting or in the field so that hypothermic treatment can be quickly instituted before significant tissue damage occurs. Methods are also disclosed for targeting brain cooling by irrigating the upper airway/aerodigestive tract, and more generalized cooling by irrigating the esophagus and/or stomach.

A heat exchanger module (HEM) and system uses a flexible substrate with one or more open channels, to which a substrate cover is bonded, thereby forming closed channels in the flexible substrate. Thermoelectric coolers (TECs) are attached to optional thermally diffusing copper squares atop the substrate cover. An interface cover is attached to the TEC tops, with a compliant thermally conductive material opposite the TECs and ultimately in contact with a patient. A liquid is passed through the closed channels, which act as thermal references for the TECs. Current is supplied by a controller to the TECs to induce TEC cooling or heating relative to the liquid. One or more temperature sensors detect the temperature of the interface cover, which are used as inputs to the control of the TEC supply current. The HEM may be used for heating, cooling, or cycling between heating and cooling for various medical uses.

Methods and devices are disclosed for providing therapeutic hypothermia using irrigation of the aerodigestive tract with cooled liquids. The disclosed method provides much more rapid therapeutic cooling to lower temperatures than could previously be achieved, thereby improving clinical outcomes. Novel cooling liquids and cooling devices are disclosed for carrying out the method. An external turbulent flow of cooling liquid may also be applied to the exterior of the head to further promote heat exchange. Multiple embodiments of devices are disclosed for performing rapid induction and maintenance of therapeutic hypothermia either in a hospital setting or in the field so that hypothermic treatment can be quickly instituted before significant tissue damage occurs. In a brain cooling device according to an embodiment of the present invention, preferably, temperature sensors are installed in one end and the other end of a cooling fluid transport tube to detect a temperature, a pressure measurement sensor is installed in a storage tank, a control unit is connected to the temperature sensors and the pressure measurement sensor, and the control unit calculates an amount of heat absorbed from a brain according to a change in pressure of the storage tank and the temperature detected by the temperature sensors. In the brain cooling device according to the embodiment of the present invention, preferably, one end of the cooling fluid transport tube and a drainage tube is bent such that adjacent drainage tubes come in close contact with each other. The brain cooling device according to the present invention is directly installed in a dura mater of a brain or a site in which an operation of incising the dura mater of the brain is performed so that heat or swelling is effectively mitigated. After use of the device ends, the brain cooling device may be removed through an operation hole which allows a cooling fluid to be introduced and discharged and penetrates a scalp and a cranium and thereby the device may be removed without a separate operation. A method of improving brain function in a hypothermic patient is provided. The method comprises administering to the patient an amount of a fibroblast growth factor 21 (FGF21) effective to increase RNA binding motif 3 (RBM3) production in nerve cells of the patient. The method is useful, for example, where the patient is or will be undergoing cardiac surgery or spinal surgery, such as requiring deep hypothermia circulatory arrest (DHCA), or is subject to an emergency preservation and resuscitation (EPR) method, or where a patient suffers from mild/moderate/severe traumatic brain injury (TBI), ventricular fibrillation cardiac arrest (VFCA), subarachnoid hemorrhage (SAH), subdural hematoma (SH), cerebral vasospasm, neonatal abusive head trauma (a.k.a. shaken baby syndrome), neonatal hypoxic ischemic encephalopathy (HIE), asphyxia cardiac arrest (ACA), treatment of spinal injury, prophylaxis in spinal surgery, stroke, and drug overdose.

It has been discovered that compositions comprising magnetically heatable entities (MHEs), therapeutic agents and optional carriers such as hydrogels can be piloted from an injection point in a blood vessel to a specific location of the blood-brain barrier (BBB) using for example, a magnetic resonance imaging (MRI) device for propelling, steering and tracking of MHEs. Hypothermic therapy headgear for patients includes a headpiece for engaging the head of the patient and a cooling member engaged to the headpiece for contacting at least one surface portion of the patient. The cooling member includes at least one cooling surface for contacting the surface portion of the patient, and at least two endothermic reaction components. The endothermic reaction components have an initial state where the endothermic reaction components are separated from contact with each other, and a treatment state in which the endothermic reaction components are placed into contact, wherein an endothermic reaction takes place and cools the cooling surface and the corresponding portion of the patient. The system of claim 1 , wherein the humidifier is operably connected to the enclosed chamber via a conduit.3. The system of claim 2 , configured to have a first state in which the humidifier delivers humidified air via the conduit to the interior of the chamber; and a second state in which the conduit is disconnected from the chamber and humidified air remains sealed within the interior of the chamber.4. The cooling garment defines chambers containing a heat transfer fluid. During operation, a user places the cooling garment in thermal communication with a body portion of a subject. As the cooling garment receives the liquid refrigerant from the pressurized source, the liquid refrigerant thermally contacts the heat transfer fluid and evaporates, thereby reducing the temperature of the heat transfer fluid. The heat transfer fluid, in turn, reduces the temperature of the body portion in thermal communication with the cooling garment. The heat transfer fluid acts to substantially evenly distribute cooling, as provided by the evaporation of the liquid refrigerant, to the body portion contacting the cooling garment to minimize localized “cold spots” within the chamber.

A cooling system includes a console and a tissue cooling device such as a head-cooling device. An operator applies the head-cooling device to the head of a patient at risk for ischemic injury. The console provides a cooling fluid to a fluid circulation space located between the cooling device and the patient's head under a positive gage pressure. Direct contact between the cooling fluid and the patient's head provides a relatively rapid induction of systemic hypothermia in the patient, thereby minimizing or preventing ischemic injury in the patient. The console also removes air from a channel disposed about an inner rim of the cooling device, using a negative gage pressure. Such removal of the air from the channel seals the rim of the cooling device to the head of the patient, including portions of the channel in contact with hair of the patient's head, and minimizes leaking of the cooling fluid beyond the rim of the cooling device.
