The assembly comprises two or more catheter tubes fused together to form a fused catheter bundle. Each tube has at least one lumen extending longitudinally through the catheter from its distal end to its proximal end and at least one of said tubes comprises a pressure resistant lumen. The tubes are fused together by use of heat & pressure generated by heat shrinkable tube slides positioned over a segment of the catheter tubes while mandrels are positioned within each tube lumen. After cooling, the heat shrinkable tube may be removed and the mandrels removed such that and the fused catheter bundle is formed. One or more of the tubes may be of a different hardness, material and/or color. A distal end of the fused catheter bundle can be split free floating, stepped or tapered tipped. The anchor includes a sloping base plate that is attached to the skin of a user for providing stability and immobility to the tubing. The plate can include an extension and cover for securing the tubing into position, and a shelf that allows the plate to extend beyond the extension to prevent skin contact with the tubing. The access valve is a T-shaped adapter for connecting the tubing to a dialysis machine tubing, and includes a flushing branch that eliminates the need to disconnect the tubing from the dialysis machine when flushing the dialysis vascular access. During treatment, an occlusion can occur at different locations in the patient line and/or the catheter. When an incremental volume of additional solution is provided to the patient line while the occlusion is present, a change in pressure results. The change in pressure depends on the dimensions and the distensibility of the non-occluded portion of the patient line. If the change in pressure, the incremental volume, the properties related to the distensibility of the patient line, and some of the dimensions of the patient line are known, the location of the occlusion can be inferred. The occlusion type can be inferred based on the determined location. 1. Disclosed is a system and method for splitting dual lumens using hot stamping. The system includes a die assembly having a heated die with a septum that is configured to split the dual lumen to form a split catheter by splitting the septum of the dual lumen. The dual lumen and the mandrels are pushed via a block into the die until the dual lumen passes through the septum within the die and two lumens are fully formed. An apparatus for delivery of a vascular arteriovenous graft comprises a tubular sleeve defining an interior lumen adapted for being slidably positioned over a tunneling instrument. Once the tunneling instrument has been advanced to a desired subcutaneous anatomical location the shaft is removed while the sleeve remains in the tissue. The sleeve has a linear slit extending from a proximal end to a point intermediate the length of the sleeve. The lumen receives the distal end of the graft and at least a portion of a cannulation chamber such that the end of the sleeve progressively expands along the slit to provide an enlarged diameter for accommodating the cannulation chamber. Longitudinal force to the sleeve during removal of the sleeve from the tissue secures the graft in the sleeve by radial compression of the sleeve for pulling the graft and the sleeve through the tunnel and deploying the vascular graft. 2. The subcutaneous delivery apparatus as recited in claim 1 , wherein the sleeve is adapted to receive substantially the entire length of the shaft between the handle and the tip.3. Apparatus for intravascular access device displacement detection, where an intravascular access device is inserted into a blood vessel through the skin at an access site. The apparatus comprises electrodes located on either side of the intravascular access device which electrodes are mobile with the intravascular access device and attachable to the skin about the access site. Apparatus for use in dialyzing a patient comprising a hemodialysis catheter comprising an elongated body having a proximal and a distal end, wherein the distal end terminates in a substantially planar distal end surface; first and second lumens extending from the proximal end to the distal end, wherein the lumens terminate on the distal end surface in first and second mouths, arranged in side-by-side configuration, and further wherein the lumens are separated by a septum; and first and second longitudinal slots formed in the distal end of the elongated body and communicating with the interiors of the lumens, the slots opening on the distal end surface; wherein the slots each have a length and a width, relative to the dimensions of the lumens and the rate of blood flow to be passed through the hemodialysis catheter, so as to minimize undesirable recirculation of dialyzed blood. 136-. A height of the body in the vertical direction can be sufficiently small such that the entire port can be implanted subcutaneously in a patient. The port can include a guidance passageway that is at least partially defined by the body and can direct an access device into a vessel of a patient when the port is attached to the vessel. A device monitors a blood path from a blood vessel access of a human subject through an extracorporeal blood processing apparatus and back to the blood vessel access. A pumping device in the blood path is operable to pump blood through the blood path from the blood withdrawal device to the blood return device. The monitoring device obtains pressure data from a pressure sensor arranged upstream of the pumping device in the blood path, and processes the pressure data for detection of a disruption of the blood path downstream of the pumping device, e.g. caused by VND (Venous Needle Dislodgement). The disruption is detected by evaluating presence/absence of cross-talk pulses at the pressure sensor, where the cross-talk pulses originate from one or more pulse generators in the extracorporeal blood processing apparatus and have propagated on a propagation path in a direction downstream of the pumping device through the blood return device, the blood vessel access and the blood withdrawal device to the pressure sensor.
