An endoscopic vessel harvesting tool uses an optics system to present of a video image to a user. As the tool is manipulated around all sides of the vessel to dissect and cut surrounding tissue and side branches, the orientation of the captured image rotates. A motion tracker (e.g., gyrosensor) in the tool detects the rotation. By detecting a rotational angular velocity of the harvester handle during the harvesting procedure, the camera view orientation is compensated before display to the user. When the handle is rotated, the detected rotation is used to provide an opposite (canceling) rotation of the camera view so that a steady orientation is presented on the display. Thus, a vertically upward direction (or any other desired reference direction) remains substantially fixed in the displayed video images.

A method of treating a vessel in a subject comprises the steps of advancing a device distally across a treatment zone in a vessel, wherein the device comprises an elongated catheter having a lumen and a distal end, and a radially expansive treatment element disposed in the lumen and configured for axial movement relative to the catheter; deploying the radially expansive treatment element proud of the distal end of the catheter to radially expand and circumferentially impress against the vessel lumen at a distal end of the treatment zone; and withdrawing the deployed radially expansive treatment element proximally along the treatment zone with the treatment element circumferentially impressed against the vessel lumen to mechanically and circumferentially denude the treatment zone of the vessel. An endoscopic vessel harvester includes a vessel keeper for capturing a target vessel within a tunnel that has been dissected along the target vessel. A cutter member cuts and cauterizes side branches while the target vessel is slidably captured in the vessel keeper. A spray nozzle carried by the vessel keeper delivers a preservative fluid via a manual control valve and a conduit. The preservative fluid is sprayed on the target vessel proximate a respective side branch immediately after being cauterized. Thus, a harvested vessel is bathed in preservative fluid prior to being actually removed from the body, enhancing endothelial preservation from cascading events.

The main components of such systems include a vessel dissector and a vessel harvester, both of which work in conjunction with a separately provided endoscope. The vessel dissector is an elongated cannula having a blunt tip for separating layers of facial around vessels. The tip may be movable, and is typically transparent to permit viewing forward of the tip using the endoscope. Internal features of the tip may reduce glare back to the endoscope. Several devices improve visibility through the tip by reducing interference from tissue or fluid on the tip. The vessel harvester also has an elongated cannula for receiving the endoscope. Several tools within the harvester permit manipulation, severing, and sealing of vessels forward of the distal end. The tool for manipulating vessels may have a low-profile for increased visibility of operation, and may be coupled to the cannula with a damping mechanism to reduce the possibility of avulsion of the vessels. An intraoral extramedullary compression-distraction device is installed which is fixed with mini-screws passing through the two cortical layers, followed by inner cortical bone fracturing, bone fragments compression and lower jaw distraction. After the retention period, bilateral sagittal-plane osteotomy of the mandible is performed with removal of compression-distraction devices and upper jaw osteotomy with its subsequent extension forward with further upper and lower jaw fixation in orthognathic position. Then, by lower jaw trapezoidal osteotomy with chin forward projection, augmentation genioplasty is performed.
