The invention utilizes a newly designed thermal filament to allow the tip of the suture removal instrument to be slipped under the stitch in order to heat and cut the stitch. 5 Current suture removal techniques utilize scissors, forceps, and/or scalpels. These techniques, which are well known in the art, are problematic because they exert tension on the stitch and are associated with patient discomfort. Small stitches add to the difficulty of suture removal because they have less suture laxity for scissor insertion. ABSTRACT A system for varying the pulse repetition rate of a pulsed laser beam includes a computer which is electronically connected to the system's oscillator and regenerative amplifier. Specifically, the computer is electronically connected to the oscillator to create a first signal which is indicative of a first pulse repetition rate for the laser beam, as it is initially generated by the oscillator. The computer then selectively divides the initial first pulse repetition rate by an integer to create a second signal which is indicative of a second pulse repetition rate. The second repetition rate is variable according to the dividing integer and is substantially lower than the first repetition rate. The Pockels cell of the regenerative amplifier is then activated by the computer with the second signal to admit pulses from the oscillator laser beam into the regenerative amplifier at the second pulse repetition rate. Thus, the output laser beam from the regenerative amplifier comprises amplified pulses having a variable second pulse repetition rate. The system may also include an attenuator which is controlled by the computer to establish the energy level of pulses in the output laser beam.

The difference between the first and second power levels and the temperature of the tissue between the first and second stylers are monitored to control the ablation of the target tissue. The supply of the first RF signal is terminated when the monitored temperature of the first stylet exceeds a first predetermined value and the supply of the second RF signal is terminated when the monitored temperature of the second stylet exceeds a second predetermined value. By adjusting the frequency and/or phase of the first and second RF signals, the relative amounts of bipolar and monopolar ablation can be adjusted for accurate control of the lesion volume. The lesion volume can also be controlled by varying the deployment length of the electrodes.

A medical probe device for reducing tissue mass in a selected portion of the body comprises a torquable catheter having a control end and a probe end. The probe end includes a stylet guide means with a flexible tip and a tip directing means extending from the control end to the flexible tip for changing the orientation of the central axis of the stylet guide means for directing a flexible stylet outward through the stylet port and through intervening tissue to targeted tissues. A stylet is positioned in the said stylet guide means. The stylet can be an RF electrode, microwave antenna, biopsy means, fluid supply means or combination thereof. Preferably, the stylet is a non-conductive sleeve having an electrode lumen and a second lumen therein, the electrode lumen terminating at a distal port in the distal end of the non-conductive sleeve, a radiofrequency electrode being positioned in said electrode lumen for longitudinal movement therein through the distal port. The medical probe device is particularly useful for removing tissue mass from the prostate and can be used for treating BPH or benign or cancerous tumors of the prostate. The device of this invention can be used in combination with a viewing scope such as a cystoscope, endoscope, laproscope and the like, being sized to extend therethrough or it can include a viewing scope. In one construction, the flexible tip comprises a metal tube with parallel spaced-apart slots extending through the tube to a continuous longitudinal section and enclosed within a flexible sleeve, whereby the tip will preferentially bend in a plane through the axis of the tube and the continuous longitudinal section.

Tabs are attached to the proximal end of the sleeve and electrode for advancing them together from the terminal end of the tube, for retracting the sleeve from the distal end of the electrode to expose a predetermined area of electrode surface, and for retracting them together into the terminal end of the tube. The electrode can also be a hollow tube, the proximal end thereof being adapted to be connected to a suction source for aspiration of tissue adjacent its distal end or it can optionally contain a fiber optic, the end thereof being enclosed within the tube and closely adjacent its distal end. In one embodiment, the electrode is a hollow electrode tube of highly flexible memory metal, preformed to have a curved memory configuration. The portion of the electrode tube and its surrounding sleeve extending beyond the distal end of the outer tube adopts the curved memory configuration, causing it to follow a curved path when extended through intervening tissue to reach a target tissue to be ablated.

A novel suture removal instrument, kit and technique are described herein. The invention utilizes a newly designed thermal filament to allow the tip of the suture removal instrument to be slipped under the stitch in order to heat and cut the stitch. Current suture removal techniques utilize scissors, forceps, and/or scalpels. These techniques, which are well known in the art, are problematic because they exert tension on the stitch and are associated with patient discomfort. Small stitches add to the difficulty of suture removal because they have less suture laxity for scissor insertion. The present invention therefore allows for more rapid suture removal with less patient discomfort and at a competitive or lower cost.

A handle is coupled to the proximal extremity of the elongate probe member. The distal extremity of he guide cannula has a bendable distal portion. The handle includes a first actuator for causing the distal portion of the guide cannula to bend at an angle to the longitudinal axis so that the opening faces the tissue and a second actuator secured to the stylet for advancing the radio frequency electrode and the layer of insulating material from the opening in the guide cannula into the tissue.

FIELD: medicine. SUBSTANCE: group of inventions relates to medicine, namely to the method for irreversible electroporation (IEP) and the system for its implementation. In this case, catheter electrodes are selected to supply IEP pulses to the organ tissue between the selected electrodes. A IEP protocol is established containing an impedance threshold. The impedance between the selected electrodes is measured. The measured impedance is compared with a given threshold. If the measured impedance is below the threshold, the IEP protocol is changed so that the new impedance threshold is below the measured impedance. Changing the IEP protocol involves changing the pulse width, pulse repetition rate and/or the number of IEP pulses that meet the specified safety criterion for the measured impedance. IEP pulses are supplied in accordance with the modified protocol. Safe ablation is provided with protection against excessive Joule heating of the tissue by maintaining the value of the supplied energy similarly to the planned value using modified parameters of the IEP pulses so that the heat generated can spread to a greater extent and the maximum temperature caused by heating decreases in order to perform ablation at a given impedance with the same clinical and thermal effect. A method and a system for destruction of an undesirable formation in a mammalian body are described. The invention is based on inserting a probe (12) into mammalian body; said probe is electrically connectable to a control unit (18) and upon bringing the probe in physical contact with the formation supplying pulses of energy from the control unit to the probe in order to destroy the formation. The invention further comprises counting amount of pulses being supplied to the probe and establishing a remaining service life of the probe by subtracting the amount of pulses being supplied to the probe from an amount of pulses corresponding to initial service life of the probe. The treatment session is automatically terminates as soon as amount of pulses being supplied to the probe approaches at least a fraction of the established remaining service life of the probe. The established remaining service life of the probe is stored for new treatment session. Delivery of pulses automatically terminates when the remaining service life is exhausted and use of such probe becomes impossible.

The method of the present invention enables pulses to be supplied to the instrument for a defined interval (T D ) after which there is a defined interval (T E ) in which no pulses are delivered. At the end of this pause pulses are again delivered to the instrument. Provision of a pause enable the surgeon to better control the operation, for example, by preventing overcutting of tissue.
