Methods and apparatus for treatment of patent foramen ovale (PFO) provide for applying energy to tissues adjacent the PFO with a catheter device to substantially close the PFO acutely. Applied energy may be monopolar or bipolar radiofrequency energy or any other suitable energy, such as laser, microwave, ultrasound, resistive heating or the like. Some embodiments of a catheter device fuirther include one or more tissue apposition members near the distal end for helping bring PFO tissues together, such as a PFO covering member, a vacuum applying member and/or the like. PFO closure via energy-based approaches of the invention may help prevent stroke, treat migraine headache, and possibly treat or prevent other medical conditions.

Apparatus and methods for closing a septal defect including a flap of tissue partially detached from a septum of a heart. A needle is advanced through a patient's vasculature within a delivery apparatus until the needle is disposed within a first chamber adjacent the septum. The needle is directed through the flap of tissue until the needle is disposed within a second chamber opposite the septum. A filament attached to an intermediate region of the needle is pulled, thereby causing the needle to pivot about the intermediate region such that the ends of the needle straddle the septal opening. A locking element is secured to the filament to secure the flap of tissue against the septum, and the filament is cut. Alternatively, the filament is biased to coil to engage the flap of tissue and secure the needle against the septum to close the opening.

A needle removal device is provided for removing needles from suturing systems. In an embodiment, the needle removal device may include a base member configured to be selectively positioned substantially adjacent a proximal portion of a suturing system. The proximal portion of the suturing system may include a plurality of needle lumens. The needle removal device may also include a plurality of needle receptacles at least partially defined by the base member. The needle receptacles may be positioned and configured to generally correspond to the needle lumens of the suturing system. The present invention provides a method of repairing a defect in an abdominal wall of a subject, including the steps of: making a small incision through a skin layer above the defect in the abdominal wall, introducing a surgical device including a deformable body through the incision to reach a preperitoneum underlying the abdominal wall around the defect, using the surgical device to identify a protruded region of the preperitoneum extending towards the abdominal wall around the defect, and flattening the protruded region of the preperitoneum and separating the protruded region of the preperitoneum from the abdominal wall with the deformable body in an expanded condition. The method results in low recurrence of abdominal wall defect, less pain, and a shorter hospital stay after surgery.

Surgical clips and surgical applicators used in performing rapid tissue closure in either minimally invasive surgeries or traditional open procedures are provided. In one example approach, a surgical clip comprises opposing sides extending from a top portion and terminating at tips positioned below the top portion. The resting position of the clip is its closed position. Edges of the top portion and the opposing sides form opposing faces perpendicular to the opposing sides. A method , comprising:introducing, into a vessel wall, a removable guidewire segment and a distal end of a body, the removable guidewire segment removably attached to a distal housing portion at the distal end of the body, the body comprising a suture element held within the body and at least one suture capture rod within the body, the suture capture rod being operatively associated with the suture element and arranged to pass the suture element through the vessel wall such that opposed portions of the suture element extend from the vessel wall;deploying, via the body, the suture element into the vessel wall;exposing, from the vessel wall and while maintaining pressure on the suture element, the distal housing portion of the body;detaching, from the removable guidewire segment and at the distal housing portion, the body such that a proximal end of the removable guidewire segment is detached and outside of the vessel wall; andattaching, to the proximal end of the removable guidewire segment and via a coupling component, a distal end of a proximal guidewire extension, wherein there is a first smooth transition at a first juncture between the proximal guidewire extension and the coupling component and a second smooth transition at a second juncture between the removable guidewire segment and the coupling component.18. Devices, systems, and methods for suturing of body lumens allow the suturing of vascular puncture sites located at the distal end of a percutaneous tissue tract. An elongated articulated foot of the device can be inserted through the penetration and actuated so that the foot extends along the lumenal axis. The foot can carry suturing attachment cuffs with one end of the cuff adapted to receive a needle, while the other end receives suture. A portion of the foot and/or lumen of the shaft can receive a portion of the suture and can include friction reducing structure that aid with movement of the sutured during removal of the cuffs from within the penetration. 1.
