An interface e.g. button 167 on the housing is moved in a first direction in order to activate one or more piercing elements 158 that are extendable in a second direction through an opening 140 of the housing. The second direction is different to the first direction e.g. non-parallel, orthogonal, oblique. By moving the interface in a direction different to the direction of the piercing elements the perception of pain may be reduced. Skin is drawn into a recess 136 in the base of the housing under vacuum. An interface 115 may be provided for manually activating the vacuum, or it may be automatic upon sensing that the device has been placed on skin.
