A device for dialysis treatment with artificial kidneys consists of a dialysis cell (3), a conveyor pump (8) for the patients blood and a conveyor pump (9) for the dialysate soln. A dosing pump (6) is situated in pipe (15) for recycling blood after the dialysis cell (3). This pump adds Ca2+ and/or Mg2+ ions from a container to the patients blood. USE/AVANTAGE - The high citrate ion content ensures that citrate ions diffuse into the blood and act in the membrane filter as anticoagulants. The citrate ions are degraded in the body by the Krebs cycle. Citrate intoxication is prevented by ensuring that the content of free ionised Ca is low and the Ca2+ is continuously added to the blood after the dialysis. Thus the use of heparin is avoided.

Systems and methods convey the blood through a gap defined between an inner surface that is located about an axis and an outer surface that is concentric with the inner surface. At least one of the inner and outer surfaces carries a membrane that consists essentially of either a hemofiltration membrane or a hemodialysis membrane. The systems and methods cause relative movement between the inner and outer surfaces about the axis at a selected surface velocity, taking into account the size of the gap. The relative movement of the two surfaces creates movement of the blood within the gap, which creates vortical flow conditions that induce transport of cellular blood components from the membrane while plasma water and waste material are transported to the membrane for transport across the membrane. Shear-enhanced transport of waste materials and blood plasma water results.

The pressure pulses cause air bubbles from the blood side of the dialyzer to be sheared off the dialyzer membrane. The air bubbles are then conducted from the dialyzer and removed from the extracorporeal circuit. Backfiltering of fluid across the dialyzer from the dialysate circuit into the extracorporeal circuit is performed in synchrony with the pressure pulses to assist in the shearing off of air bubbles from the blood side of the dialyser membrane.

A system, method and apparatus for performing a renal replacement therapy is provided. In one embodiment, two small high flux dialyzers are connected in series. A restriction is placed between the two dialyzers in the dialysate flow path. The restriction is variable and adjustable in one preferred embodiment. The restriction builds a positive pressure in the venous dialyzer, causing a high degree of intentional backfiltration. That backfiltration causes a significant flow of dialysate through the high flux venous membrane directly into the patient's blood. That backfiltered solution is subsequently ultrafiltered from the patient from the arterial dialyzer. The diffusion of dialysate into the venous filter and removal of dialysate from the arterial dialyzer causes a convective transport of toxins from the patient. Additionally, the dialysate that does not diffuse directly into the patient but instead flows across the membranes of both dialyzers provides a diffusive clearance of waste products.

Hot water disinfection of dialysis machines, including the extracorporeal circuit thereof

A dialysis machine is disclosed that incorporates a water treatment module, a dialysate preparation module having a dialysate circuit, and an extracorporeal circuit including a dialyzer and arterial and venous blood lines that connect to the patient. The machine accomplishes on-line disinfection of all the fluid circuits of the machine, including the water treatment module, the dialysate preparation module and extracorporeal circuit, the arterial and venous lines, and the dialyzer, by circulating water heated to a high level disinfection temperature (e.g., 80 degrees C.) through the fluid passages of the machine for a sufficient time (such as an hour) to achieve high level disinfection. After the dialysis session is ended, the patient connects the arterial and venous lines to ports in a disinfection manifold that places the arterial and venous lines into fluid communication with the heated water. Thereafter, the machine is ready to disinfect the fluid circuits including the extracorporeal circuit. In a preferred embodiment, the ports of the disinfection manifold ports include recessed features that permit the hot water to circulate on the interior and exterior surfaces of the arterial and venous line connectors, thereby insuring that the surfaces of connectors themselves are also disinfected.

A method is described for priming a dialyzer placed in an extracorporeal circuit of a hemodialysis machine in situ. The method comprises the steps of substantially filling the extracorporeal circuit and the blood side of the dialyzer with fluid (such as dialysate, RO water, saline, etc.), and inducing pressure pulses in the fluid. The pressure pulses cause air bubbles from the blood side of the dialyzer to be sheared off the dialyzer membrane. The air bubbles are then conducted from the dialyzer and removed from the extracorporeal circuit. Backfiltering of fluid across the dialyzer from the dialysate circuit into the extracorporeal circuit is performed in synchrony with the pressure pulses to assist in the shearing off of air bubbles from the blood side of the dialyzer membrane.

The connector is pressed into sealing contact with an O-ring disposed in the interior region of the first chamber, thereby preventing any leakage around the connector and port. During dialysis, the method continues with the following steps: conducting a third measurement of the intensity of light of the light source that passes though the chamber, the chamber containing dialysate solution that may contain the patient's blood, with the light source in the first condition. Next, the state of the light source is changed from the first condition to the second condition. A fourth measurement of the intensity of light passing through the solution is made. A suitable processor calculates an attenuation coefficient indicative of the presence of blood in the dialysate solution during dialysis from the first, second, third and fourth measurements.

In another embodiment, a separate fluid leakage detector is placed at the base of the extracorporeal circuit module for detection of leakage in situ in the extracorporeal circuit, while the catchment chamber and sensor in the base of the machine detect leaks from the other modules.

Methods and apparatus for increasing the accuracy of ultrafiltration in a dialysis machine and decreasing the time required for dialysis. The machine inclused a substantially non-compliant tank for mixing and storing a batch of dialsyate. In a preferred form, the tank is a hollow vessel made from a lightweight and chemically inert material, such as polypropylene, reinforced on its exterior. The machine further includes an ultrafiltration pump for pumping fluid from an extracorporeal circuit to an ultrafiltration tank commensurate with the removal of water from the patient. The pump is calibrated with the volume of fluid in the tank. The reduction in the time to conduct hemodialysis of the patient is achieved by introducing the patient's blood into the arterial line and conducting the blood to the blood pump, measuring the pressure of the blood in the arterial line, increasing the pump rate of the blood pump, and in the event that the pressure measured in the arterial line drops below a predetermined threshold value, slowing the pump rate of the blood pump.
