An expansible tunneling apparatus and associated methods for creating an anatomic working space for a surgical procedure. Various apparatus permitting observation during tunneling and balloon dissection are disclosed. In one embodiment, a tubular member has a bore extending therethrough and an open distal end. A lip is formed in the distal end of the tubular member to capture the distal tip of a laparoscope inserted into the tubular member to permit observation of the procedure during tunneling to a desired location and balloon inflation. According to the present invention, improved methods and apparatus are provided for providing cushioning and other ergonomic surfaces on devices requiring the patient or tissue to be compressed, such as radiography machines, fluoroscopy units, mammography units and the like. In particular a radiolucent pad element is provided for releasable attachment to at least one surface of a compression device to be used under x-ray, for example, during mammography. The pad element of the present invention can be disposable or constructed to be reusable and in some cases may be applied directly to the patient's breast. Furthermore, a cushioned paddle is provided wherein the compression paddle and the cushion can be separately or integrally formed.

One embodiment of the defibrillator electrodes (36) includes a flexible conductive mesh (38) and non-conductive mesh (40). Another embodiment of the defibrillator electrodes includes a flexible conductive mesh, a non-conductive mesh and an insulator therebetween. A third embodiment of the defibrillator electrode compensates for the shape of a human heart. Methods for implanting the defibrillator electrodes include rolling an electrode and inserting the rolled electrode into a subxiphoid opening while thorascopically observing the insertion and manipulation of the defibrillator electrode.

A surgical clamp or tissue retractor is disclosed having gripping surfaces from which extend resilient filaments. The distal ends of some of the resilient filaments abut against the engaged vessel or tissue to restrict movement of the vessel or tissue relative to the gripping surfaces. In the preferred embodiment, the resilient filaments are arranged in rows and oriented at particular angles relative to the gripping surfaces.

This invention is an improved tissue localizing device for fixedly yet removably marking a volume of tissue containing a suspect region for excision. This invention also encompasses methods for deployment of the localizing device and its excision along with the marked tissue volume. At least one locator element (200) is deployed into tissue (10) and assumes a predetermined curvilinear shape to define a tissue border containing a suspect tissue region along a path. The locator element path preferably encompasses the distalmost portion of the tissue volume (20) without penetrating that volume (20). Multiple locator elements (200') may be deployed to further define the tissue volume along additional paths defining the tissue volume border that do not penetrate the volume (20).

Surgical clamp pad with interdigitating teeth

Opposable surgical clamp members having opposable resilient pads with wedge-shaped and cylindrical protrusions extending from the surfaces of the pads and the use of such members for occluding vessels and other tubular body structures are described. The protrusions are arranged such that when the members are moved toward one another, the protrusions of one pad interdigitate with the protrusions of the other pad. Upon engagement with a vessel, portions of the vessel are forced into the interdigital spaces providing for improved gripping of the vessel. The protrusions also resist lateral or transverse movement of a clamped vessel relative to the pads. The existence of through holes under the pad surface alters the relative resiliency of the pad, allowing for an overall pad resiliency that minimizes trauma to a clamped vessel while allowing the local resiliency of the protrusions themselves to be of a hardness to avoid excessive deflection and retain the desired shape and gripping ability.

A compression device for a mammography unit includes a compression plate positionable in an x-ray field of the mammography unit, and a radiolucent foam pad removably attached to the compression plate that includes an impermeable exposed surface for contacting tissue disposed on the pad within the x-ray field. The pad includes an adhesive layer on the first surface, or a hook and eye fastener for removably attaching the pad to the compression plate. The pad is attached to the compression plate in the x-ray field of the mammography unit, a breast is compressed using the compression plate, a mammogram of the compressed breast is obtained, the pad producing no significant visual artifact on the mammogram, and thereafter the pad is removed.
