The handle has a connector that connects to a proximal end of an endoscope, a shaft portion that rotatably connects to a proximal end of the connector, and a manifold that is slidably received by the shaft portion. The manifold connects to the sheath and allows for insertion of a radial ultrasound probe, through access port 56, and of a medical tool, through angled side port 54, into the sheath. A stopping device 62 in the manifold restricts deployment of the medical device, which may be a sampling (biopsy) needle. The sheath may include a telescoping tube to prevent buckling. The distal end of the sheath may have an exit ramp (78, figure 4) to laterally deflect the needle.

Systems and methods for controlling stiffness of a needle or catheter

An arrangement for stiffening a flexible tube such as a needle 40 or catheter has a stylet device of variable stiffness. The device includes a stylet 46 with a bulbous distal end (54, figure 4). A proximal length of the stylet extends slidably through a push tube (80, figures 6 and 9). A compression element such as a coil spring 50 extends between the distal end of the push tube and the bulbous distal end of the stylet. When the push tube is moved distally by movement of a handle arrangement it compresses the spring and thereby stiffens the stylet device. The handle arrangement may include a partially threaded cavity which receives a threaded shaft of an adjustment knob. The stylet may be secured to the handle by a pin or set screw. The stylet device may be placed inside a flexible needle system for sampling bronchial tissue.

It is difficult to determine the direction of a lung tumour which is off centre of the airway. The intraluminal tip 120 of bronchoscope has: a sheath 122 with lumens (182, 184, fig 15) lined with a liner 180, the lumens for receiving an ultrasound probe and a medical tool (such as a sampling needle), and a ramp deflector 140 for guiding the medical tool to protrude from the tip at an angle. There is a further sleeve 130, 132 forming an ultrasound probe cavity within the tip. There may be orientation pins 128 made of ultrasonic reflective material which cast a shade (92, fig 9) to indicate the orientation of the probe, such that the orientation of the tumour can be determined, and that sample can be collected from the correct target. The nose 126 may be rounded for atraumatic tissue interaction. The nose has a port 152 for receiving ultrasound gel.
