Devices, systems, and methods are provided for managing suture filament during a tissue repair procedure. One exemplary embodiment of an anchor insertion tool includes a handle, an elongate shaft extending distally from the handle, and a ring disposed around the elongate shaft. The ring can be configured to slide along a length of the shaft, and can be configured to engage a filament extending from an anchor removably coupled to the shaft's distal end during a suture anchor insertion procedure. In some embodiments, the ring can include one or more slots for receiving a filament and maintaining a tension applied to the filament. The ring can also include features that prevent it from rotating with respect to the shaft, such as a boss formed on a ring's central opening that engages a slot formed in the shaft. Other devices, systems, and methods for suture anchor insertion are also provided.

Systems, devices and methods to improve safety and efficiency in an operating room comprise providing a suture package that holds new suture needles and needle receptacles for storing used needles. The devices can be safely worn for the surgeon to self-dispense new suture needles in the near surgical field and to secure the used needles into a needle trap or a needle retainer located on his extremity, on his operative instruments or on the surgical drapes. The device may provide automated and/or simplified needle counting both during use and after removal from the surgical field. The device may be configured for ergonomic and efficient use so as to minimize the actions and motions of the surgeon to dispense and secure the needle.

A dispense r for sutures is disclosed that includes a first and second rigid and substantially flat panel, each comprising a central opening and an edge. The panel edges cooperate to retain the first panel against the second panel to form a dispenser of substantiall y two dimensional character. The disclosed dispensers are thin and thus require le ss storage and shipping space. Methods of loading dispensers are also disclosed. By winding the suture material in a flat spiral around a set of winding pins between the two panel s, the disclosed dispensers can be easily and accurately filled, yet the resulting coiled suture can be withdrawn via the central openings without twisting or kinking the suture material.

A molded suture retainer is provided fox retaining and storing sutures. Both absorbable and nonabsorbable sutures fabricated from natural or synthetic materials can be advantageously retained and stored therein. The retainer is characterized by a wide spiraling oval passageway with minimal convolutions covered by a cover sheet. A package is disclosed for containing a plurality of combined surgical suture devices, particularly of the double armed type having a pledget and used for cardiovascular surgical procedures. A backing panel has a protective cover adhesively affixed to one side and defines a plurality of straight elongated suture tracks for reception of the flexible suture portion therein. An aperture is positioned at each end of each track to facilitate reception of the flexible suture portion into the track and drawing a vacuum therein to assist drawing the flexible suture portion into the track. Preferred embodiments include separate features such as a foam needle park or an end flap configured to support the a suture needles beneath tabs. A surgical suture retainer retains the suture in an epitrochoidal, or hourglass configuration. One embodiment includes an insert member mounted to one of several foldable connected panels of the suture retainer. The insert member preferably has first and second wings which fold over to form a suture retaining pocket. The suture is held in a looped configuration having an hourglass shape. Another embodiment employs tabs cut from one of the panels to retain the suture in the hourglass shape. Synthetic absorbable sutures are filled with a stabilizing agent, preferably a mixture of glycerol and calcium lactate, and inserted into a retainer having a narrow convoluted passageway. The retainer is inserted into an open foil pouch with a package stabilizing element such as a paper sheet filled with stabilizing agent. The package containing the suture, retainer and package stabilizing element is sterilized, aerated, equilibrated to a relatively high moisture level, and sealed. Bends and kinks in the suture are prevented so as to provide a synthetic absorbable suture having highly desirable out of package hand and feel. A molded two-piece suture package is disclosed which encloses and defines an oval channel in which sutures may be wound. The oval channel has two semicircular end sections connected by straight side sections. An opening in the channel is provided at the juncture of a straight section and an end section, permitting sutures to be withdrawn from the channel at the end of a straight section. In a preferred embodiment the interior wall of the channel is formed by alternating engaging sections formed in the respective package pieces. To prevent the wrapped suture from becoming entrapped when the two pieces are engaged, the top and bottom of the channel is alternately raised and lowered in alignment with the alternating wall sections, causing the wound suture to bridge those areas where it may become entrapped between the mating package pieces. Suture dispensers for retaining armed sutures are disclosed. The dispenser disclosed is comprised of two flat panels of material that each have a central opening. A needle park is provided so that the needle is visible in and accessible though the central openings. In preferred embodiments, the needle park is made of a thin sheet of clear material. Several alternate needle park designs are also disclosed. The two panels are preferably locked together using edge locks in the form of corresponding slits and tabs formed on the panels themselves. In addition to being inexpensive, easy to manufacture and easy to fill, the disclosed dispensers is much thinner than current designs and thus requires less storage and shipping space. Methods of loading dispensers are also disclosed . By winding the suture material in a flat spiral around a set of winding pins, the disclosed dispensers can be easily and accurately filled, yet the resulting coiled suture can be withdrawn via the central openings without twisting or kinking the suture material. In certain embodiments pressure on the central portion of the panels permits winding a spiral without the need for a central winding core. In other embodiments, pins are used to both align the panels and provide a winding guide for the suture material. a suture winding apparatus is also disclosed. The winding apparatus has upper and lower plates and a base member. A pair of opposed arcuate members extends downwardly from the lower plate.

Retainers for needled surgical sutures

A suture retainer includes a first panel and foldably connected second through fifth panels. When all of the panels are folded, the retainer has the approximate size and shape of the first panel, and the suture is contained within the folded panels. Separator facilities on the first panel prevent suture strands from becoming tangled. The separator may constitute a pleated low friction sheet with the strands contained between the plats, or it may constitute one or more sinuous cuts in the first panel which define tabs for holding the strands. Sutures are withdrawn from a site at one end of the first panel. The second panel is foldably located at the other end of the first panel and maintains suture strands within the boundary of the first panel. The third panel is foldable about a side of the first panel to overly the folded second panel, the separator and the sutures and to complimentarily overly the first panel. A holder, such as a foam strip on the first panel and near the withdrawal site, holds the suture strand to maintain the needle strand attachment point at the withdrawal site. Facilities normally cover the needlepoints at the withdrawal site. These facilities may constitute either a tab on the third panel, which covers the needlepoints when the third panel is folded, or a removable sheath on the needles. Tabs on the third panel fit into slits at the boundary of the first and fourth panels to keep the third panel folded.

An improved retainer for surgical sutures is provided which includes a suture retaining compartment comprising a suture winding panel with the panel having a pair of holding panels foldably connected to longitudinal edges of the suture winding panel. The holding panels are foldably connected to the winding panel by at least three gussets formed at the edges of the suture winding panel. The gussets at the ends of the foldable connection prevent frictional compression of the wound suture at the corners of the retaining compartment, and the central gussets or gussets engender a convex curvature to the holding panels, thereby creating a low friction volumetric compartment which retains the suture in place. The suture retainer also includes a needle holding means mounted at the intended location of the needle. In illustrated embodiments of the present invention the holding means is angularly disposed with respect to the longitudinal dimension of the retainer to afford ease of needle withdrawal, and in some embodiments the holding means is located on a foldable needle park panel. In some embodiments the retainer includes an insert against which the suture is wound. This insert provides several features, including preventing snagging of the suture on the needle holding means during suture withdrawal, and protection of the needle point. The insert may also include a flap to prevent the suture from snagging on a corner of the insert during withdrawal.
