Systems, devices, and methods are provided for securing soft tissue to bone. One exemplary embodiment of a surgical repair construct includes an anchor, a filament having a snare formed on one end and a collapsible loop on another end, and a suture having a plurality of stationary protrusions formed therein. The suture is configured to be coupled to detached tissue and have its ends passed through an opening in the snare. The snare can be collapsed around the suture such that at least one of the suture protrusions is proximal of the collapsed snare. The anchor can be disposed in bone and the filament coupled thereto. Accordingly, collapsing the snare around the suture couples the tissue to bone, and applying tension to a tensioning limb of the filament can collapse the collapsible loop to incrementally tighten and secure the tissue to bone. Other exemplary systems, devices, and methods for use with soft tissue repair are also provided.

The guide assembly is adapted to dispense at least one barbed insert from the dispenser opening into the filament winding assembly for winding a plurality of filaments (284,308) around the at least one barbed insert for making a braided barbed suture. The passage way of the dispenser opening is adapted to allow longitudinal movement of the barbed insert relative to the passageway while simultaneously preventing twisting movement of the barbed insert relative to the passageway. As the barbed insert is being dispensed, the barbed insert dispenser opening is selectively rotatable for imparting rotation to the barbed insert as the filaments are wound about the barbed insert. The passageway may be an elongated slit having a greater width than height.

Described are various embodiments of an improved endoprosthesis that includes a generally cylindrical graft portion that extends along a longitudinal axis to define a flow passage and a plurality of stent hoops. The plurality of stent hoops are connected to the graft portion and disposed in a spaced apart relationship along the longitudinal axis. There is at least a first suture disposed along on the outer surface of the main body. The first suture connects at least two of the spaced apart stent hoops together. The first suture also includes unidirectional barbs (or multidirectional barbs) configured to reduce movement of the main body with respect to a direction of blood flow in an artery.

Methods for producing absorbable self-retaining sutures that have high tensile strengths and pronounced sheath-core structures wherein the sheath is harder than the core are also provided. An integrated needle and cannula assembly for plastic surgery including a syringe connected to a blunt-tipped cannula where the blunt-tipped cannula is inserted into the hollow annular space of a hypodermic needle. In operation, the medical practitioner penetrates into the patient's skin using the hypodermic needle and then slides the blunt-tipped cannula through the annular space of the needle. A transporter assembly can be interposed between the syringe and the overlapping needle/cannula. The transporter assembly may include a retractable inner needle tube attached to the hypodermic needle and an external transporter housing which connects the syringe to the cannula through a cannula connector. Once the blunt tipped cannula is under the patient's skin, the needle is preferably withdrawn from the patient's skin to allow sub-dermal substances to be injected using only the blunt-tipped cannula. Withdrawal of the needle can be assisted with either a pull spring or push spring. In an alternative embodiment, a sharp-tipped trocar inside a cannula is used to penetrate the patient's skin. In a further alternative embodiment, a barbed surgical thread may be attached to said cannula to perform thread-lift procedures.

Self-locking knotless sutures and associated methods are disclosed. A system for securing tissue includes a monofilament and a braid having a fixed end and a capture end. A first end of the monofilament is connected to the fixed end of the braid. A second end of the monofilament opposite the first end is a free end. The capture end of the braid has a first opening and a second opening. The monofilament and braid are structured and arranged for the free end of the monofilament to be inserted into the first opening, passed through a tunnel formed by the braid, and passed out of the second opening, all while moving in a first direction. After insertion of the monofilament into the braid, the braid prevents movement of the monofilament in a second direction opposite the first direction to prevent the monofilament from being pulled out of the braid.
