Apparatus and a method of inserting spinal implants is disclosed in which an intervertebral space is first distracted to restore the normal angular relationship of the vertebrae adjacent to that disc space. An extended outer sleeve having extended portions capable of maintaining the vertebrae distracted in their normal angular relationship is then driven into the vertebrae adjacent that disc space. Spinal plates with additional features to improve the stability of the interface between the plate and the underlying bone. A bone plate may include one or more sharp ridges along the periphery of its underside. When attached to bone, the ridge digs into the bone and increases stability. A bone plate may alternatively or additionally include one or more holes for optional spikes, which may be inserted once the plate is attached to the bone. By separating the spikes and including them as an optional component, the plate may enhance stability while reducing or eliminating the chance of the spike injuring the patient. Furthermore, bone screws may incorporate alternating notches and ridges into the head of the screw. The notches and ridges may interface with a set screw, thereby preventing rotation and loosening of the screw.

A method of using an interbody spinal implant by implanting the spinal implant into a patient in need of the spinal implant. A bone plate may include one or more sharp ridges along the periphery of its underside. When attached to bone, the ridge digs into the bone and increases stability. A device for the selective fracture of the inner lining or tunica intima of a blood vessel without severing of the surrounding outer layer or tunica adventitia of the vessel is disclosed. The device comprises a combination of an anvil member having a dull blade affixed thereto which acts as one blade of a guillotine which descends upon the vessel caught between it and a hammer member of the guillotine. The hammer member is spring loaded through connection with a slider arm which extends upward toward the spring and a grip for the fingers of the user. There is provided a guide for the anvil and its blade which guide is aligned and connected with the central rod which extends between the slider arm and the spring which causes impact on the hammer member of the guillotine. The central rod acts as a core for two springs which surrounds the same. The first spring is referred to as a proximal compression spring and the second spring is referred to as distal actuator spring which is located peripherally to the proximal compression spring. The action of the first compression and the release of these two springs causes a shock to extend down the central rod or core into the slider arm and hence into the hammer portion of the guillotine. This shock action follows a first closing of the opening between the anvil and the hammer of the guillotine and is of such controlled momentum that it causes the dull blade of the anvil to fracture the inner layer of the blood vessel without severing the outer muscular layers of the blood vessel.

Instrument and method for removing stones (calculi) from the kidneys, the bladder and other parts of the body. A stone is captured in a basket at the distal end of an elongated tubular member and broken into pieces while it is held by the basket. The stone is broken up by a hammering action provided by a reciprocating shaft which extends through the tubular member into the basket and is driven toward the stone by a spring. The stone is removed from the body by withdrawing the tubular member from the body with the pieces of the stone in the basket. Since the tip of the shaft impacts upon the stone inside the basket, the instrument is safe in that the reciprocate tip cannot strike the tissue outside the basket.

The device of generating of an one-impulse mechanical shock wave contains a shock agent for high rate striking blows on an agent of generating of a shock wave. The shock agent is set in motion under the influence of force of expansion of the compressed gas. Gas is submitted before producing every impulse of a shock wave to an agent of accumulation, which is supplied with gas of high pressure from independent agents of storage by means of extenders of gas and food and hermetic sealing agents. A drill is then passed through the hollow sleeve removing disc and bone in preparation for receiving the spinal implant which is then inserted through the sleeve.

An improved method and instrumentation for performing spinal surgery, including discectomy, interbody fusion and rigid internal fixation of the spine, from the lateral aspect of the spine is disclosed. The surgical procedure can be performed through a very small incision. The instrumentation of the present invention, all of which is inserted from a lateral position into the spine in the preferred embodiment, comprises a guide pin, a distractor, an extended outer sleeve, an inner sleeve an adjustable drill and an implant driver. The distractor of the present invention is driven into the disc for spacing apart and realigning the adjacent vertebrae. It further functions as an alignment rod for inserting the extended outer sleeve which is a hollow tubular member capable of maintaining said spacing and alignment of two adjacent vertebrae and defines a protected space through which subsequent instruments which may include, but are not limited to, a drill and a diameter reducing inner sleeve may be passed, as well as a spinal implant. The remainder of the surgical procedure consisting of the removal of spinal material across the disc, fusion, and rigid internal stabilization via the implant may all be performed via the closed space within the extended outer sleeve.

There are provided various embodiments of medical instruments to perform knee surgery. In one embodiment a finned platform for mounting a cutting block is provided. The finned platform can be used don either a femor or tibia to allow for the proper cuts when performing a knee surgery. In another embodiment, a tibial trial is shown having a fin. The fin is useful to reinforce the bone to reduce the risk of fracture during bone preparation. In another embodiment, a reamer is provided having a plurality of cutting flutes. It may be desirable to utilize a guide with the reamer to allow the reamer to cut a noncircular portion of the tibial bone. In yet another embodiment, a plurality of fixation pegs are provided on the tibial implant to allow for easy removal of such implant if a revision surgery becomes necessary.
