In an electrosurgical system which is operable within the frequency range of from 100kHz to 50 MHz and which has a handheld instrument with an elongate instrument shaft, the distal end of the shaft carries a bipolar electrode assembly for use in a conductive fluid field. The electrode assembly has an active electrode and, set back from the active electrode, a return electrode coated with an insulative dielectric layer. The insulative layer may be made of a plastics or a ceramic material and acts as the dielectric of a capacitor when the electrode assembly is immersed in the conductive fluid. One of the effects of the insulative layer is to provide a more uniformly distributed electric field over the return electrode than is obtained with an exposed return electrode, yielding improved tissue desiccation and vaporisation characteristics. A catheter has a multifunctional “virtual” tip electrode with a porous substrate and a multitude of surface microelectrodes. An acoustic marker mounted on the catheter emits a sonic wave when electrically excited. A central processing unit creates a graphical representation of the internal structure, and super-imposes items of data onto the graphical representation at locations that represent the respective plurality of locations within the internal structure corresponding to the plurality of items of data. The present invention provides a surgical cutting instrument having a blade portion and cutting edge therefor with electric input elements located near the cutting edge for cutting the tissue and cauterizing the surfaces of the incision, thereby allowing surgery to be more rapidly performed. This is accomplished in accordance with the illustrated embodiments of this invention by providing electrodes of opposed polarity, applied to the blade, near the cutting edge. With an electrical potential applied, no current will flow between the electrodes and no heat is produced unless the electrode gap is bridged by a conducting medium such as a high conductivity physiological fluid. Heat is then generated by electric discharge below an arcing threshold in all areas where the blade is in contact with moist tissue. Little electric discharge or heat occurs elsewhere. Moreover, if movement of the blade is halted, heat generation will substantially diminish as the cut tissue becomes dry as a result of cauterization. A process according to the present invention may include hemostasis of intact tissue and selected use of the instrument for cutting other materials compatible with the process.

The closed configuration is adapted for clamping and coagulating a target tissue while the apparatus is operating in the sub-ablation mode, while the open configuration is adapted for ablating the target tissue via molecular dissociation of tissue components. A method of the present invention comprises clamping a target tissue or organ with an electrosurgical probe. A first high frequency voltage is applied between the active electrode and the return electrode to effect coagulation of the clamped tissue. Thereafter, a second high frequency voltage is applied to effect localized molecular dissociation of the coagulated tissue. The present invention allows the ablation or modification of the target tissue with minimal or no damage to surrounding, non-target tissue.

The method of manufacturing the ablation device includes using commercially available stretchable sheet material or providing a mandrel or support base and coating the mandrel or base with an uncured emulsion. The emulsion is then cured to form a bladder-like coating. The coating is thereafter removed from the mandrel or base to form a bladder. Electrons and temperature sensors are formed on or secured to the bladder during or after the curing process and a fluid delivery tube is secured to the bladder for inflating the bladder.

Such cooled plate electrodes are capable of directing electrical energy through an intermediate tissue and into fascia while the cooled electrode surface prevents injury to the intermediate tissue, particularly where the electrode surfaces are cooled before, during, and after an intermittent heating cycle. Ideally, the plate electrode comprises an electrode array including discrete electrode surface segments so that the current flux can be varied to selectively target the fascia. Alternatively, chilled 'liquid electrodes' may direct current through a selected portion of the bladder while also cooling the bladder wall, an insulating gas can prevent heating of an alternative bladder portion and the adjacent tissues, and/or ultrasound transducers direct energy through an intermediate tissue and into fascia with little or no injury to the intermediate tissue.

An electrical connection to the hollow tube is provided for supplying the jaws with a cautery current.

An electrosurgical medical device and technique for creating thermal welds in engaged tissue that provides very high compressive forces. In one exemplary embodiment, at least one jaw of the instrument defines a tissue engagement plane carrying first and second surface portions that comprise (i) an electrically conductive material and (ii) a positive temperature coefficient (PTC) material having a selected increased resistance that differs at each selected increased temperature over a targeted treatment range. One type of PTC material is a doped ceramic that can be engineered to exhibit a selected positively sloped temperature-resistance curve over about 37° C. to 100° C. The 70° C. to 100° C. range can bracket a targeted “thermal treatment range” at which tissue welded can be accomplished. The engineered resistance of the PTC matrix at the upper end of the temperature range will terminate current flow through the matrix. In one mode of operation, the engagement plane cause ohmic heating within tissue from Rf energy delivery tissue PTC matrix is heated to exceed the treatment range. Thereafter, energy density in the engaged tissue will be modulated as the conductivity of the second portion hovers within the targeted treatment range to thereby provide optical tissue heating for purposes of tissue welding.

Surgical instruments of the invention are provided with distal ends having a clevis and two discrete investment cast end effectors. One end effector is stationary relative to the clevis, and the other moves or rotates about a transverse pin of the clevis. The rotating end effector includes a first hole through which the transverse pin extends, and the surgical instrument includes an actuation mechanism for causing the rotating end effector to rotate about the transverse pin. The actuation mechanism preferably includes a push rod which is either directly coupled to the proximal end of the rotating end effector or coupled by a staple element. On the other hand, the stationary second end effector, while including a first hole through which the transverse pin extends, has a boss which preferably engages a hole in the clevis and is not coupled to the actuation mechanism. With the clevis and second end effector being coupled at two locations, the second end effector is not free to rotate about the clevis pin, but is held stationary. Other manners of holding the second end effector stationary are to provide the clevis with a boss, and the stationary end effector with a second hole into which the boss extends, or to provide another pin which extends through one arm of the clevis and the stationary end effector only.

A non-metallic end effector for use in an endoscopic surgical tool includes a metallic core for strength and for providing a selected electrode surface on the end effector. Selectively conductive end effectors are manufactured by insert molding a plastic or ceramic or other non-conductive body around a metallic or otherwise conductive core. The conductive core is exposed on a selected portion of the working surface of the end effector and extends through the non-conductive body of the end effector for coupling to an electrical source. Selectively conductive end effectors of this type can also be manufactured by coating a cast end effector member with a non-conductive polymer or by metallically plating a non-conductive ceramic end effector member. By extending the conductive core of a selectively conductive end effector member to the pivot hole of the member, electrical coupling can be made through the clevis and tube of an endoscopic instrument. By extending the conductive core to the actuation bore of an end effector member, electrical coupling can be made through the push rod or other actuation member in an endoscopic instrument. Using both techniques, a bipolar selective cautery end effector can be provided on an endoscopic instrument.

The distal portion also have cut-outs which increase in width as they extend distally. The lung clamp end effectors are at least two inches long from their respective pivot through-holes to their respective distal ends.

The invention provides a variety of devices and methods for applying gentle resistive heating of these and other tissues to cause them to contract without imposing significant injury on the surrounding tissue structures. Alternatively, heat-applying probes are configured to heat tissue structures which comprise or support a patient's urethra. By applying sufficient energy over a predetermined time, the tissue can be raised to a temperature which results in contraction without significant necrosis or other tissue damage. By selectively contracting the support tissues, the bladder neck, sphincter, and other components of the urinary tract responsible for the control of urinary flow can be reconfigured or supported in a manner which reduces urinary leakage.

An interior of the jaw carries a conductive material or electrode that is coupled to an Rf source and controller. In an exemplary embodiment, the variably resistive matrix can comprise a positive temperature coefficient (PTC) material, such as a ceramic, that is engineered to exhibit a dramatically increasing resistance (i.e., several orders of magnitude) above a specific temperature of the material. In use, the engagement plane will apply active Rf energy to captured tissue until the point in time that the variably resistive matrix is heated to its selected switching range. Thereafter, current flow from the conductive electrode through the engagement surface will be terminated due to the exponential increase in the resistance of variably resistive matrix to provide instant and automatic reduction of Rf energy application. Further, the variably resistive matrix can effectively function as a resistive electrode to thereafter conduct thermal energy to the engaged tissue volume. Thus, the jaw structure can automatically modulate the application of energy to tissue between active Rf heating and passive conductive heating of captured tissue to maintain a target temperature level.
