This can be done by exposing the lung to an electromagnetic field causing dielectric or eddy current heating. The invention is particularly useful for treating emphysema as the diseased areas in emphysema patients have reduced blood flow. The diseased area will heat up rapidly while the healthy tissue will be cooled by the blood flow. This is particularly effective for treating emphysema because of the low mass of the lungs and the high blood flow. To avoid heating of surrounding organs the direction of the electromagnetic energy may be switched in a way it always passes through lungs but only intermittently passes through adjacent organs.

Devices and methods for monitoring the temperature of tissue at various locations in a treatment volume during fluid enhanced ablation therapy are provided. In one embodiment, an ablation device is provided having an elongate body, at least one ablation element, and at least one temperature sensor. The elongate body includes a proximal and distal end, an inner lumen, and at least one outlet port to allow fluid to be delivered to tissue surrounding the elongate body. The at least one ablation element is configured to heat tissue surrounding the at least one ablation element. The at least one temperature sensor can be positioned a distance away from the at least one ablation element and can be effective to output a measured temperature of tissue spaced a distance apart from the at least one ablation element such that the measured temperature indicates whether tissue is being heating to a therapeutic level.

The invention provides for a medical instrument ( 100 ) comprising: a high intensity focused ultrasound system ( 122 ) for sonicating a target region ( 139 ) within a subject ( 118 ); a light source ( 158 ) for exciting a temperature sensitive fluorescent dye, wherein the light source is configured for coupling to an optical fiber cable ( 148 ) of a urinary catheter ( 140 ); a light source ( 160 ) for measuring fluorescence ( 190 ) emitted by the temperature sensitive fluorescent dye, wherein the light source is configured for coupling to the optical fiber cable; a memory ( 178 ) for storing machine executable instructions ( 180 ); and a processor ( 174 ) for controlling the medical instrument. Execution of the machine executable instructions cause the processor to: receive ( 200 ) a sonication plan ( 188 ) descriptive of a sonication of the target region; measure ( 202 ) the fluorescence using the light sensor; calculate ( 204 ) a bladder temperature ( 192 ) using the fluorescence; and generate ( 206 ) sonication commands ( 194 ) using the sonication plan and the bladder temperature, wherein the sonication commands are adapted for controlling the high intensity focused ultrasound system to sonicate the target region.

A translucent or transparent fluid, such as saline, can be pumped into the imaging hood until the fluid displaces any blood, thereby leaving a clear region of tissue to be imaged via an imaging element in the deployment catheter. An electric current may be passed through the fluid such that it passes directly to the tissue region being imaged and the electrical energy is conducted through the fluid without the need for a separate ablation probe or instrument to ablate the tissue being viewed.

An electrosurgical instrument has a handle and a body which position and close a jaw about a tissue site for simultaneously cutting and sealing relatively large tissue structures. The jaw includes an electrosurgical cutting member, which may be a blade or wire, against which tissue is biased along a cut line, and a clamping assembly that clamps a region adjacent to or surrounding the cut line. The clamping assembly includes sealing electrodes for heating the region and welding tissue along the side of the cut as the cutter parts the tissue. The clamping assembly preferably has first and second clamping jaws extending in parallel to grip the tissue as tension is released by the cut, allowing dependable and complete sealing of the cut ends over an extended time while the tissue is immobilized. Sealing electrodes formed of thermally conductive material may be covered by a thinner coating or thermally non-impeding or heat transfer cover, which preferably includes a material to assure biocompatibility and prevent sticking. The handle, body and jaw may be configured in the shape of a hemostat to allow simple positioning, or may be configured with an elongated body and a mechanism that operates the jaw through a stationary or telescoping handle for endosurgical use through a small incision. The blade may extend entirely to the tip, or the clamp assembly may extend forward of the blade. An energy ratio input element may attach to an energy source to apportion electrosurgical energy between the cutting member and the sealing electrodes. The element may include a pair of transformers, or a transformer with a split secondary or a tapped secondary winding, with the primary configured for connection to the output of an RF scalpel drive console. The windings of the transformer match impedances and power requirements of the different cutting and sealing electrodes to coordinate effective tissue sealing and welding with the cutting operation.

Implant apparatus and method for effecting a controlled heating of tissue within the region of dermis of skin. The heater implants are configured with a thermally insulative generally flat support functioning as a thermal barrier. One surface of this thermal barrier carries one or more electrodes within a radiofrequency excitable circuit as well as an associated temperature sensing circuit. The implants are located within heating channels at the interface between skin dermis and the next adjacent subcutaneous tissue layer such that the electrodes are contactable with the lower region of dermis. During therapy a conformal heat sink is positioned against the skin above the implants and a slight tamponade is applied through the heat sink to assure uniform dermis contact with electrode surfaces. An adjuvant may be employed to infiltrate dermis to significantly lower the thermal threshold transition temperature for dermis or dermis component shrinkage.

The injected fluid can provide deeper and narrower conduction paths and resulting lesions. Radio frequency ablation can be performed at the same time as the fluid injection, using the injection device as an electrode, or subsequent to the fluid injection, using a separate device. In some methods, the injected fluid is a protective fluid, injected to protect tissue adjacent to the desired lesion line. Fluid delivery can be endocardial, epicardial, and epicardial on a beating heart. The present methods find one use in performing maze procedures to treat atrial fibrillation.

Thereafter, current flow from the conductive electrode through the engagement surface will be terminated due to the exponential increase in the resistance of variably resistive matrix to provide instant and automatic reduction of Rf energy application. Further, the variably resistive matrix can effectively function as a resistive electrode to thereafter conduct thermal energy to the engaged tissue volume. Thus, the jaw structure can automatically modulate the application of energy to tissue between active Rf heating and passive conductive heating of captured tissue to maintain a target temperature level.

A thulium fiber laser system can be used to treat tissues based on the ability for quick changes between laser pulses. For example, to treat stones in a tissue, a long pulse having low peak power can be used to create bubbles in front of the stone (calculi), then follow a series of shorter pulses and higher peak power can be used to break the stone. The sequence can be repeated to maintain large bubble formation, with the long pulse characteristics changed to accommodate for the changes in the tissue. A fluorescent sensing assembly can be used to detect the tissue conditions for selecting the conditions of the thulium fiber laser.
