When an acetabular cup used in hip replacement surgery is to be affixed to the acetabulum using screws, the drill guide instruments of the invention are used to position a drill bit so that the screw holes drilled in the acetabulum are properly placed for optimum implantation of the acetabular cup. The drill guide is locked into the acetabular cup by spring loaded pins which position the guide over holes in the cup which accommodate the screws. The guide is used to position the drill bits and allow drilling. After drilling the guide is removed.

Methods and apparatus of the present invention provide for orthopaedic total replacement of the hip while substantially preserving muscles and soft tissues around the hip joint resulting in reduced healing time and decreased risk of dislocation following the procedure. In an exemplary embodiment, the acetabulum is prepared and fitted with a prosthetic acetabular cup 702 and a prosthetic femoral head 1102 is fitted into the prosthetic acetabular cup 702. An intramedullary rod 1502 is inserted longitudinally into the femur. A prosthetic femoral neck 3102 is then inserted from a point along the side of the patient's body and into the side of the femur and through a pre-existing lateral bore in the intramedullary rod 1502 and through the remainder of the femur to join the prosthetic femoral head 1102. This prosthesis has excellent safety against break and suffers little wear. It is simple to make and to implant.

The intermediate volume between the inner shell (1) and the outer shell (2) thus narrows in the direction of taper (13) so that the fastening element (8), when tightened in the direction (13), wedges itself with its head (9) between the boundary surface (1i) of the inner shell (1) and the groove (11) of the outer shell (2). Play between the inner thread (7a) and the outer thread (8a) is preferably provided so that, when the fastening element (8) is tightened, the force transmission region (9b) of the head (9) seats against the boundary surface (1i) of the inner shell (1) and the force transmission region (9a) seats against the groove (11) of the outer shell (2). The inner shell (1) thus lies wedged in the outer shell (2) due to the fact that the fastening element (8) transmits a radially oriented force between the inner shell (1) and the outer shell (2).

The present invention pertains to the field of medical techniques and can be used in traumatology and in orthopedics for placing hip-joint endoprostheses. The present invention relates to a cup for the cotyloid notch in a complete hip-joint endoprosthesis, wherein said cup comprises a thin-wall conical ring having a self-tapping thread on its outer surface as well as an opening in its upper base. The cup also includes a conical polymeric insert tightly fitted into the ring, wherein said insert has an open hemispherical recess formed at its lower base for mounting the prosthesis head, as well as an annular protrusion that engages with a corresponding groove formed on the inner surface of the ring. The cup further includes a porous disc which is fixedly mounted on the upper base of the insert. This invention is characterised in that the upper bases of the conical ring and the disc are spherical and in that they share a common centre. The outer surface of the porous disc, which engages the bone tissue, has a radius which is either 1.01 or 1.10 times larger than the radius of the sphere of the conical ring, the pores on said outer surface being open. The pores on the opposite surface and on the lateral surface are essentially closed. The groove on the inner surface of the conical ring is formed at the smallest distance possible from the lower base thereof. This invention provides a solid fixation, good stability and anatomic properties, reduced requirements concerning the mounting precision, a strong adhesion of the whole surface of the cup to the bone tissue as well as a good bone integration.
