A retractor and a surgical tool are positioned within a cannula, and a dissection cradle of the retractor is positioned at the distal end of the cannula. The retractor includes a first portion with an axis approximately parallel to the axis of the cannula and a second portion with an axis that can be skewed relative to the axis of the cannula. The dissection cradle is located at the distal end of the second portion of the retractor, and may include two substantially parallel, spaced legs with the retractor shaped in a loop between and in a plane skewed relative to the axes of the legs, and with the loop directed away from the surgical tool. Thus, in operation, a surgeon locates a vessel and side branch of interest and extends the retractor to cradle the vessel in the dissection cradle. Once cradled, the retractor may be deflected to urge the vessel away from the axis of the cannula to isolate the side branch for exposure to the surgical tool. Removable, transparent tips are selectively positioned at the distal end of the cannula for performing dissection and transection via a single cannula. Additionally, the tips are configured to align the apices of the tips with the central axis of the endoscope to maximize the visual field through the tips via the endoscope. Increasing the effective aperture of an ultrasound imaging probe by including more than one probe head and using the elements of all of the probes to render an image can greatly improve the lateral resolution of the generated image. In order to render an image, the relative positions of all of the elements must be known precisely. A calibration fixture is described in which the probe assembly to be calibrated is placed above a test block and transmits ultrasonic pulses through the test block to an ultrasonic sensor. As the ultrasonic pulses are transmitted though some or all of the elements in the probe to be tested, the differential transit times of arrival of the waveform are measured precisely. From these measurements the relative positions of the probe elements can be computed and the probe can be aligned. 1. Construction of probes can vary by medical application. That is, a general radiology probe can contain multiple transducers that maintain separate physical points of contact with the patient's skin, allowing multiple physical apertures. The body wall engaging element includes an equatorial portion facing into the central hole. The apparatus also includes a flexible lifting element that has a flexible portion capable of passing through the laparoscopic incision. The flexible portion is attached to the equatorial portion of the body wall engaging element and extends from the equatorial portion towards the broad lifting face. The flexible lifting element also includes an adapter, connected to part of the flexible portion remote from the equatorial portion of the body wall engaging element, that receives the external lifting force and transfers the external lifting force to the flexible portion.

In order to render an image, the relative positions of all of the elements must be known precisely. A calibration fixture is described in which the probe assembly to be calibrated is placed above a test block and transmits ultrasonic pulses through the test block to an ultrasonic sensor. As the ultrasonic pulses are transmitted though some or all of the elements in the probe to be tested, the differential transit times of arrival of the waveform are measured precisely. From these measurements the relative positions of the probe elements can be computed and the probe can be aligned.

Each of these steps may be performed wholly from the inside of the stomach and thus can eliminate the need for any surgical or laparoscopic intervention. After one or more plications is formed, medical devices may be coupled to the plication(s) for retention within the stomach.

Construction of probes can vary by medical application. That is, a general radiology probe can contain multiple transducers that maintain separate physical points of contact with the patient's skin, allowing multiple physical apertures. A cardiac probe may contain only two transmitters and receivers where the probe fits simultaneously between two or more intracostal spaces. An intracavity version of the probe can space transmit and receive transducers along the length of the wand, while an intravenous version can allow transducers to be located on the distal length the catheter and separated by mere millimeters. Algorithms can solve for variations in tissue speed of sound, thus allowing the probe apparatus to be used virtually anywhere in or on the body.

One of the legs supporting the retractor is hollow (1100) and includes a spray nozzle (1104) disposed in the distal end of the retractor to form an irrigation system. The spray nozzle on the retractor may be positioned to wash the endoscope lens, or upon selective extension of the retractor out of the cannula, the spray nozzle may be positioned to direct the spray of irrigation fluid at a remote surgical site.
