A hot tip catheter assembly (3) is disclosed which resolves atherosclerotic plaque buildup in vivo. The catheter has a heater (12), a cap (14), thermocouple, power leads (22a, 22b), thermocouple leads (20), and a central distal lumen (17) to position the catheter within the artery. The catheter tip has a thin, non-adhesive coating (25) of a hard, heat-conducting material. The thermocouple is used to continuously evaluate the temperature at the tip of the catheter, and the temperature is then regulated by a computer controlled feedback system. The catheter can completely melt the buildup without damage to the artery by direct contact with the plaque, without use of balloon catheter angioplasty. Radiofrequency (RF) heating applicator, located at the distal end of a coaxial line catheter, produces deeper and more uniform heat dissipation. The active applicator element is a conductor helix fed via the coaxial line. The applicator has provisions for interception of intracardiac electrogram signal. A cardiac ablation system using the above catheter, ablates cardiac tissue responsible for ventricular tachycardia. The ablation system provides means to monitor intracardiac electrograms and to control the RF power. A variation of the helical applicator can be used in a hyperthermia system for treatment of malignant tumors.

A second coat of electrically insulating, water insoluble flexible polymer material is disposed on the first coat. The second coat comprises one or more through openings at or near its front end. An electrosurgical device capable of both: (i) generating a radiative microwave field to perform either blood coagulation or sterilization, and (ii) dispensing a liquid to a target area, e.g. adrenaline for the treatment of peptic ulcers. Using microwave energy allows controlled coagulation which is unaffected by changes in tissue impedance during coagulation, and which can achieve a high level of coagulation within a given time. The device comprises a probe tip having a coaxial transmission line that includes a hollow inner conductor having a dielectric material formed on an inner surface thereof. A liquid-conveying channel is located inside the dielectric material. The probe tip includes a conductive element coupled to receive microwave energy from the, coaxial transmission line, the conductive element forming a radiating antenna structure for emitting a microwave EM field outwardly from the probe tip.

When the guard is in the second position, the distal end of the guard is in contact with the tissue, which prevents current from arcing between the first electrode and the second electrode without passing through the tissue.

An acoustic imaging system for use within a heart has a catheter, an ultrasound device incorporated into the catheter, and an electrode mounted on the catheter. The ultrasound device directs ultrasonic signals toward an internal structure in the heart to create an ultrasonic image, and the electrode is arranged for electrical contact with the internal structure. A chemical ablation device mounted on the catheter ablates at least a portion of the internal structure by delivery of fluid to the internal structure. The ablation device includes a material that vibrates in response to electrical excitation, the ablation being at least assisted by vibration of the material. The ablation device may alternatively be a transducer incorporated into the catheter, arranged to convert electrical signals into radiation and to direct the radiation toward the internal structure. The electrode may be a sonolucent structure incorporated into the catheter, through which the ultrasound device is arranged to direct signals. An acoustic marker mounted on the catheter emits a sonic wave when electrically excited. A central processing unit creates a graphical representation of the internal structure, and super-imposes items of data onto the graphical representation at locations that represent the respective plurality of locations within the internal structure corresponding to the plurality of items of data. A display system displays the graphical representation onto which the plurality of items of data are super-imposed.

The expandable device has spaces therein through which the blood can flow when the expandable device is in an expanded position. Leads are in contact with the electrodes and extending through the flexible elongate tubular member. In a preferred embodiment, there is provided a conductive non-stick electrode for use with a hemostatic tissue cutting scalpel wherein, a first layer of conductive material having a textured working surface for contacting tissue is adherently deposited along the cutting edge of the scalpel, and a second coating of non-stick material is deposited thereon at least partially filling the textured surface, so that portions of the conductive material are exposed and the electrode is thereby conductive along said working surface but non-sticking while in contact with tissue.

A surgical cutting instrument includes an electrically heated cutting edge and an automatic control system for maintaining the cutting edge at a constant high temperature for sterilizing the blade, cutting tissue, and cauterizing the incised tissue to reduce hemorrhage from the cut surfaces of the tissues (hemostasis).

The present invention provides a surgical cutting instrument having a blade portion and cutting edge therefor with electric input elements located near the cutting edge for cutting the tissue and cauterizing the surfaces of the incision, thereby allowing surgery to be more rapidly performed. This is accomplished in accordance with the illustrated embodiments of this invention by providing electrodes of opposed polarity, applied to the blade, near the cutting edge. With an electrical potential applied, no current will flow between the electrodes and no heat is produced unless the electrode gap is bridged by a conducting medium such as a high conductivity physiological fluid. Heat is then generated by electric discharge below an arcing threshold in all areas where the blade is in contact with moist tissue. Little electric discharge or heat occurs elsewhere. Moreover, if movement of the blade is halted, heat generation will substantially diminish as the cut tissue becomes dry as a result of cauterization. A process according to the present invention may include hemostasis of intact tissue and selected use of the instrument for cutting other materials compatible with the process.

Electrosurgical apparatus employing constant voltage and methods of use

Methods and apparatus are provided for use in performing electrosurgery, wherein a power supply supplies an electrosurgical instrument with a low voltage power signal having a substantially constant peak-to-peak voltage that is independent of the load impedance and which has a crest factor near unity. A power supply is provided to supply a voltage waveform in accordance with the methods of the invention. A clipping circuit is also provided to permit practice of the methods using conventional electrosurgcial generators and previously known electrosurgical instruments, wherein the voltage amplitude and waveform is modified to the desired regime.

Electrosurgical generator adaptors

Adaptors are provided for use with electrosurgical bipolar instruments and standard commercially available electrosurgical generators that reduce coagulum buildup and sticking on electrosurgical instruments. A first type of adaptor converts a high voltage, high crest factor output voltage waveform typical of conventional electrosurgical generators to a more desirable lower voltage, lower crest factor regime. A second type of adaptor limits the peak open-circuit voltage supplied by a conventional electrosurgical generator to an electrosurgical instrument.
