The instrument further has a hammer in the distal end of the shaft and moveable relative to the anvil for crimping the suture fastener therebetween and received in the expanded receiving face. A method for treating a heart , the heart having a first chamber bordered by a septum and a wall , wherein the chamber comprises a left ventricle of the heart , the heart having a second chamber separated from the first chamber by the septum , the method comprising:providing a perforation device having a distal tip that is configured to deliver electrosurgical energy to heart tissue to enable the perforation device to penetrate through the heart tissue;penetrating, via the perforation device, the septum at a first location selected for deployment of an implant;penetrating, via the perforation device, the wall at a second location selected for the deployment of the implant;controllably steering the perforation device between the first location and the second location; anddeploying the implant by affixing a first anchor of the implant adjacent the penetration of the septum, affixing a second anchor of the implant adjacent the penetration of the wall, and applying tension between the first anchor and the second anchor;wherein the tensioned members are tensioned sufficiently to bring the wall and septum into engagement, and thereby effectively exclude a region of the wall and septum from the left ventricle with scar tissue extending along the excluded region.22. A suture cinch, cinch applicator, and cinch loader for loading the cinch into the cinch applicator are provided and permit re-use of the applicator with multiple cinches during an endoscopic procedure. Systems and methods for closing a tissue defect are disclosed. The systems may include a subcutaneous guide that is placed transcutaneously between two skin access sites and a self-locking strap that may be advanced into the body through a first access site via a first needle and then passed to a second needle. The strap may be withdrawn with the second needle through a second access site to the outside of the body. The strap may be withdrawn from the first needle, leaving the strap placed through both access sites and across the defect. As the subcutaneous guide is withdrawn from the body it pulls the captured strap such that both ends of the strap protrude from the first access site, allowing the strap to be tightened around the defect. Devices for closure of a tissue defect are also disclosed.

Procedures may be performed on the heart after the installation of a mitral valve fixation device.
