Hip replacement joint

The hip replacement joint has an outer support shell (1) which is fused to the bone structure and a separate inner shell (2) which forms the socket for the hip joint. The inner shell is located in the outer shell via two conical taper profiles, with the slope of the outer taper of the inner shell steeper than that of the inner taper of the outer shell. This ensures that the contact region (4) is nearer to the support structure of the outer shell. The tapers are between 18 deg and 20 deg with the arc difference between the tapers between 0' and 20'. The outer shell has one or more recesses (10) into which a tool can be placed to prise the inner shell away from the outer shell, for replacement.

The device has three main parts. It has a main screw (6) which has rounded head (8) with a hexagonal socket in it. The screw passes through a separate lower part and the underside of the screw head seats in a hole in the lower part. The rounded underside of the screw head matches the shape of the hole in the lower part so that a good fit occurs. On the side wall of the hole is a cylindrical section which has an internal thread. A separate top section to the implant has a corresponding internal thread which fits into the thread in the hole section and a hexagonal socket head.

An intramedullary rod 1502 is inserted longitudinally into the femur. A prosthetic femoral neck 3102 is then inserted from a point along the side of the patient's body and into the side of the femur and through a pre-existing lateral bore in the intramedullary rod 1502 and through the remainder of the femur to join the prosthetic femoral head 1102. The methods and apparatus include structures and techniques for fixing the prosthetic femoral neck 3102 in relation to the intramedullary rod 1502.

It comprises an inner body and an external dish divided into a polygon by meridional cuts, its polar area being parallel to the equator. For example, one embodiment creates at least two axes of rotation through a super-spherical space carved out of the inner surface of the cup, with a cup-cable connecting two points along the super-spherical space, and a perpendicular head-cable looping around the head and the cup-cable within the super-spherical space, and with the head-cable residing in a groove. Another embodiment creates two axes of rotation through a combination of a 1) swivel with lever and/or 2) cable attached to a circular track (with the knob or ring around cable facing inward or outward), both of which have the center of the head as the center of rotation. The third axis may be created by a horizontal swivel and by wiggle room.

Methods and apparatus of the present invention provide for orthopaedic total replacement of the hip while substantially preserving muscles and soft tissues around the hip joint resulting in reduced healing time and decreased risk of dislocation following the procedure. In an exemplary embodiment, the acetabulum is prepared and fitted with a prosthetic acetabular cup 702 and a prosthetic femoral head 1102 is fitted into the prosthetic acetabular cup 702. An intramedullary rod 1502 is inserted longitudinally into the femur. A prosthetic femoral neck 3102 is then inserted from a point along the side of the patient's body and into the side of the femur and through a pre-existing lateral bore in the intramedullary rod 1502 and through the remainder of the femur to join the prosthetic femoral head 1102. The methods and apparatus include structures and techniques for fixing the prosthetic femoral neck 3102 in relation to the intramedullary rod 1502.

An acetabular prosthesis assembly includes an acetabular shell component and an acetabular bearing component. The shell component includes a concave inner wall having a tapered surface with multiple anti-rotation slots defined therein. More particularly, the sleeve allows the screw head to move longitudinally such that the screw is prevented from damaging an inner member of the prosthesis system that provides an articulation surface for a corresponding prosthetic joint component. The screw threads provide initial mechanical fixation of the prosthesis to bone and the surface treatment provides long-term fixation through bone or other tissue ingrowth and remodeling.

An acetabular cup prosthetic device comprised of an outer shell component and a inner bearing insert and the method of implanting the acetabular cup prosthetic within a patient. The outer surface of the shell component for the present invention acetabular cup has a plurality of regions which conform to the curvature of at least one ellipsoid. To implant the shell component, the acetabulum of a patient is spherically reamed to a size slightly smaller than that of the outer surface of the shell component. As the shell component is inserted into the spherically shaped acetabulum, the interior surface of the acetabular cavity deforms to assume a substantially ellipsoidal curvature, producing an interference fit between the rim region of the shell component and the upper peripheral rim region of the acetabulum. The remaining surface regions of the acetabulum conform substantially to the ellipsoidal shape or shapes defined by the remaining surface regions of the shell component so that surface contact is established without the development of ejection forces therebetween.

In one embodiment of the invention, a method is provided including the steps of preparing an opening in the bone sized to receive a prosthesis therein and then inserting an apertured temporary implant into the opening. A fluent material is then injected into the temporary implant so that the material exudes from the apertures into contact with the bone surrounding the temporary implant. The temporary implant is then removed to make way for implanting the prosthesis into the bone opening. The fluent material can be a biological, biocompatible or bioactive material, such as a composition adapted to permanently fill voids and irregularities in the bone or a composition adapted to promote bone growth. In another embodiment, an implant assembly includes an apertured sleeve adapted to be permanently implanted within the bone opening, but providing apertures for injecting a fluent material into the opening. The sleeve defines a cavity for receiving the distal portion of a prosthetic joint element, which can be fixed to the sleeve.

The internal fluid communicating passageways convey synovial fluid from a joint space to the articulation surface of an artificial joint. Accordingly, artificial joint components formed according to the present invention are less likely to develop wear debris. The implantable articles are formed by a casting process from casting molds that are prepared using a three dimensional printing technique.

In a total knee replacement (TKR), the use of a cushion element provides better wear characteristics than polyethylene (“poly”) alone. Since a metal-on-metal, metal-on-ceramic, or ceramic-on-ceramic articulating surface has better wear characteristics than metal on poly, the invention essentially provides cushioning for metal/ceramic-on-metal/ceramic joint replacements. It also allows the use of elastomers for their cushioning properties rather than their surface wear and tensile strength characteristics. The contained compressible elements could also be used as a cushion below polyethylene components, polyethylene over metal components, unicondylar knee replacements, patellar components, and prosthetic components for other parts of the body, including the hip, elbow, shoulder, wrist, and ankle.

This invention provides a prosthetic acetabular component that comprises two constructs, one being a metal base construct that engages the bone and the other being a polyethylene bearing construct that attaches to the metal base construct and articulates with a femoral stem prosthetic component. The metal base construct is composed of two different metals, one of which engages the bone surface and the other of which engages the polyethylene bearing construct. Each of these metals is selected so that its characteristics are well suited to its particular function. The first metal is selected so as to provide a superior bone-engaging face, while the second metal is selected so as to provide a superior polyethylene-engaging face. By combining the different material characteristics of two different metals in the metal bone construct, it is possible to simultaneously form a superior bone-engaging face and a superior polyethylene-engaging face.
